We accept all major insurances except for Medicare/Medicaid, Fidelus and United Healthcare Empire Plan at this time. 

What should I bring for my first appointment?  

• Prescription from your physician authorizing us to treat you
• Insurance card or insurance information
• New Patient Forms (click for link)
• List of medications you are currently taking
• Depending on the part of the body being treated, you may wish to bring your own shorts, t-shirt or appropriate clothing for exercise. However we do have some available for your use.
